As far in advance as you can, fill out all relevant forms, accessed through our secure patient portal which is linked in our confirmation emails and texts. If you are unable to log in or if the link has expired, please contact us as soon as possible.

1. New patients: please fill out all forms. 
   
2. Patients scheduled for a checkup: please update your child's medical history if it is has been more than 6 months.
   
3. Patients scheduled for treatment: please sign your child's treatment plan. If you have any questions about the proposed treatment, please contact us to schedule a phone call with Dr. Kristina before your appointment.
   
4. All returning patients: please let us know before your appointment if your insurance, address, phone number, email, or any other information has changed. ​

What are we doing to keep everyone safe? 

1. We're following all the guidelines laid out by the CDC, ADA, CDA, and SFDPH. 
2. Please continue to reschedule your child's appointment if they are sick.
3. We have Medical Grade HEPA air filters that filter 99.9% of all particles 0.1 microns and larger in diameter in every room and our building does not have central air.  
4. Any family with a member who has experienced any COVID-19 symptoms or has interacted with anybody positive for COVID-19 is not seen at our office until they have been cleared by their physician or test negative.
