Records Request Form | USPS Mail - SF Pediatric Dentistry

To request your child(ren)'s records be sent to you through USPS Restricted Certified Mail and to inactivate them in our system, please fill out the following form. This service is only available within the US. If you've moved out of the US, please choose one of the email options instead.

Postage fee: $10.35+ per child. Postage may vary depending on the weight of the letter. We will email you an invoice through Square and send the records within two business days after receiving payment.

Note: We will be using restricted certified mail to send your child's records, meaning that the addressee must sign for the letter. We will address the envelope to the parents or guardians listed below. If you list two people, only one of you will have to sign for the letter, not both.
